How to Assist Applicants with Completing Electronic Signature

Warning

Assistance requests cannot be re-opened by the applicant for electronic signature if they were in submitted or verification status before November 10, 2020.  To address assistance requests in SUBMITTED or VERIFICATION status at the time the e-signature function went live, program administrators will need to perform one of the following actions.

Decision Tree
Which form of certification do you have (or want to get) from the applicant?
  • Verbal
  • Signed Paper
  • Add Button
  1. Open applicant record.
  2. Contact applicant via telephone.
  3. Begin or Continue Assistance Request.
  4. Scroll to the certification section. 
  5. Read aloud each line of certification and release of information. (If not already checked, check each box as applicant consents.)
  6. Check box next Verbal confirmation of release of information and certification obtained
  7. Save assistance request.

  1. Open applicant record.
  2. Begin or Continue Assistance Request
  3. Scroll to the DOCUMENT section. 
  4. Upload a signed release of information and certification form
  5. Scroll to Certification section. 
  6. Check box next to Signed paper release of information and certification on file
  7. Save assistance request
